Okay, I have a question about this quest. I was wondering if you can do the F.A.T.E.'s on another class other than the one you picked the quest up for and still receive the crystals you need. Does anyone know?

Yes. Definitely. I picked it up at 60brd, and got crystals as 58ast

I cannot recall but is there a level requirement for teleportation? My friend has been to all 3 city-states and still cannot teleport.

There's no minimum level requirement to teleport, no. You learn telepot as like the very first quest in the game. The 'Close to Home' quest, which sends you around your starting city and has you attune with the aetheryte there, should also unlock the ability to teleport.
Can your friend use the 'Return' spell? If he can, then there's something a bit wrong.. might want to contact a GM or something :\

You're correct, it doesn't. We added the icon to make it stand out more, whereas all that the gear in-game shows is:
"Sanction: Critical blah blahblah"
The information is the same, even though the icon is added by us.
